The show begins with a short (5 second) sketch to "prepare" the viewer for the rest of the show. One of these beginning sketches first shows a view of the Milky Way through a telescope while Andy voices his amazement. The camera pulls back to reveal that the telescope is, in fact, pointed to a picture of the Milky Way, not the actual galaxy.

After the initial sketch, the show's intro begins, set to a truly ludicrous theme song. The show then proceeds to the more involved sketches. Examples of these sketches include Andy pretending to know random people and doing other bizarre things, such as dressing up his kittens like Jesus and Satan, or playing with his dog Wubbie. He also orders delivery food from places and plays jokes on the delivery man, such as making fake coupons, or putting shoes in pizzas and asking how they got there. He also names the pizza Larry and starts beating it up.

In every episode, Andy is wearing the same clothes: a maroon plaid shirt and navy blue jeans. In some sketches (such as his Muti sketch, where he pretends to be a nerdy kid who can't pronounce the letter "s" correctly) he tends to wear various other habiliments. He also wears different wigs and accessories as the sketches call for them. Outside, he usually wears a black zip-up hoodie sweatshirt.

At the beginning of each new sketch, the sketch name is displayed in white, animated, hand-drawn text that wiggles spastically on a black screen, sometimes accompanied by pictures. At the beginning and end of commercial breaks, a short sketch ensues within the confines of a "spikey" word bubble, the rest of the background being white. The spikes are animated in the same manner as the text, and the legend The Andy Milonakis Show is animated in the lower left-hand corner.
