What makes the Brown Medical School unique is that the majority of its students enter through a program called the Program in Liberal Medical Education (PLME). Prospective students apply to this eight-year program, considered one of the most selective programs of any university in the nation, before they begin their undergraduate education. Each year, approximately 60 students matriculate out of an applicant pool of about 1,600. Admitted students earn a Bachelor's degree in their concentration of choice after their fourth year and automatically enter the medical school to pursue their M.D. degree.

The PLME approach has revealed advantages and disadvantages. For instance, the school secures students of extremely high caliber (as evidenced by the acceptace rate) directly from high school, presumably leading to higher standards of education. On the other hand, the lack of an MCAT score requirement or other normal medical school application procedures makes the average figures low in comparison to competing medical schools (the Ivy League and other high-caliber institutions), creating debate about how reliable those statistics are for defining educational quality.

Recently, the Medical School has begun to accept applications from graduates of other colleges for the first time in almost twenty years.
