The BMJ differs from most other academic or medical journals because it functions as the "in house" journal of the BMA, as well as front line medical research. The BMJ publishes news, research, education, obituaries, reviews, fillers, anecdotes and career focus stories.

It is considered to be one of the top five general medical journals; the others being the New England Journal of Medicine, the Journal of the American Medical Association, The Lancet and the Canadian Medical Association Journal.

In the past all content of the BMJ was freely available online; however, it recently (2005) changed to a subscription model. Content is free for the first week after publication, thereafter, it is available only to those with a subscription. Of the major general medical journals, only the Canadian Medical Association Journal is free access (i.e. no registration/login is required).
