The protagonist is Sugar Bear (played by Shaggy), a streetwise cop from San Francisco who speaks in rhyming couplets and whose name is always accompanied by a trumpet riff. The New York Chief of Police (Brennan) brings Sugar Bear to New York to take care of Big Baby Sweets (Violent J), a mob don who controls the entirety of the city's criminal underworld. With the "help" of the wildly inept patrolman Harry Cox (Williams) - as well as some timely assistance by the ghost of Dolemite - Sugar Bear sets off to bring down Sweets's evil empire. This task is made rather complicated by Sweets's entourage: his ineffectual right-hand men Big Stank and Li'l Poot (played by Twiztid), his silent enforcer Hack Benjamin (Jumpsteady), a pair of stealthy magic ninjas, and Cactus Sac (Foley).

Although designed specifically with Juggalos in mind, the film has also found an audience with fans of raunchy, offbeat humor. A second installment, supposedly named "Big Money Rustlas", was rumored to be in production but has not yet been released.
